Supporting Documents

Has your claim been submitted yet? Perfect! To ease and speed up the processing thereof we require your supportive documents. Subject to the nature of claim, we may need the following:

General items: lost or damaged

  1. Receipt (if the item has been repaired or replaced).
  2. Quote for replacement.
  3. Quote for repairs.
  4. In the case of damage: A report from the technician stating the factors the damage may be attributed to, as well as costs involve to fix it.

General items: theft

  1. Receipt (if the item has been repaired or replaced).
  2. A comprehensive list of stolen items.
  3. Quotation to replace items that has been reported missing to the police. All items listed must be accompanied by a detailed description.
  4. Case number.
  5. Name of the police station where the incident was reported to.
  6. The name of the investigation officer and his/her telephone number.

Damage to your cellphone

  1. A damage report with estimated costs for repairs from the technician. Details about the cellphone must also be present on this report.

Stolen cellphone with a SIM card

  1. The cellphone number of the SIM card.
  2. The case number and name of the police station where the theft has been reported to.
  3. The phone must be blocked by your service provider. Written confirmation as proof thereof is obligatory. Your service provider will assist you in this regard.
  4. Information on the SIM card, such as the size (e.g. 8, 16, 31 Gig).
  5. IMEI number of the stolen phone.
  6. Printed quotation confirming the costs to replace the phone.

Breaking and entering of your house

  1. Alarm report from the security company confirming that the alarm was activated (only applicable if your policy requires any alarm specifications).
  2. A comprehensive list of items you are claiming.
  3. Written quotation stating the costs to replace items. PLEASE NOTE THE QUOTE MUST BE TO THE REPLACE THE EXACT MODEL, OR THE SUCCESSIVE MODEL IN CASE THE STOCK HAS BEEN DISCONTINUED.
  4. Case number, name of the police station the incident was reported to as well as the name of the investigating officer.
  5. Valuation certificate if you have it at your disposal.
  6. Any proof of purchases of the items that has been stolen.

Only my laptop has been stolen!

  1. Please provide a detailed description of the laptop and supplemented equipment that has been stolen.
  2. Be specific in your description regarding the following: Brand, Model, RAM, memory and CPU speed of the hard drive.
  3. Case number, name of the police station the incident was reported to as well as the name of the investigating officer.
